Transcript Request Form

Current students requesting a transcript for college application purposes, please contact the College Counseling Office. All other students, alumni, and past students should submit the transcript request form below.

If you are applying to another school, and will need multiple transcripts sent throughout the school year, please fill out a separate form each time you are requesting a transcript to be sent.
